The inspection shouldn’t be the moment you discover what your service looks like from outside.

By the time an assessment begins, your service has already been producing evidence for months.

We examine the trail between what your service says it does, what leaders believe is happening, what staff actually do, what people experience and what your records demonstrate.

Because when those things stop agreeing with each other, that is usually where CQC questions begin.

Training? Done.Competence? Different question.

Twenty-four certificates in a folder can prove that twenty-four people attended training.

They cannot prove that those people can recognise deterioration, respond to risk, apply the Mental Capacity Act, challenge poor practice or make a sound decision at 2.17am when there is no manager standing beside them.

We look beyond completion rates and ask the question that matters:

A care plan can describe exemplary support. We help you find out whether the person actually receives it — and whether CQC will see it too.

That means looking beyond polished documentation to staff knowledge, daily records, risk management, medicines, behavioural support, restrictive practice, choice, independence and the experience of the person receiving care.

Because care should survive contact with reality.

Action plan? Closed.Problem? Apparently immortal.

A completed action does not automatically equal improvement.

The audit was repeated.
The policy was updated.
The staff were reminded.
The box turned green.
Three months later, the same problem is back.

We test whether improvement has actually changed systems, management oversight and frontline practice — and whether there is evidence that the change lasted.

Closing the action is easy. Closing the loop is governance.

Recording an incident is the beginning of governance, not the end of it.

What happened?
Why did it happen?
What was missed beforehand?
Was the response proportionate?
Were the right people informed?
Did anything actually change afterwards?
